DISCLOSURE NOTICE AND AUTHORIZATION FOR BACKGROUND CONSUMER REPORTS

DISCLOSURE
In connection with your application for employment and for other employment purposes, Quest Utility Construction Inc. may seek background information about you from a consumer reporting agency. This information will be in the form of a consumer report, including information concerning your employment history, criminal record, education, qualifications, motor vehicle record, and/or credit history.
 

These reports may be obtained at any time after the Company receives authorization from you, including any time during the period of your employment if the Company hires you. Consumer reports may include any written, oral or other communication of information by a consumer reporting agency bearing on your suitability and qualifications for employment, including experience information and the reasons for termination of past employment. Further, information from various Federal, State, local and other agencies about your past activities may be requested in connection with a consumer report. ExpressNet, or another consumer reporting agency, will obtain the reports for the Company.

 

Before any adverse action is taken, based in whole or in part on the information contained in the consumer report, you will be provided a copy of the report, the name, address and telephone number of the consumer reporting agency, and summary of your rights under the Fair Credit Reporting Act. You have the right to inspect any files the consumer reporting agency may have on you upon your request and the provision of proper identification. For California, Minnesota or Oklahoma applicants only, you may request a copy of the consumer report, if one is obtained by the Company, by checking the appropriate box.
 

For a California applicant, if you checked the box in the preceding paragraph, a copy of the consumer report will be sent to you within three (3) days of the Company receiving the report.

AUTHORIZATION
I acknowledge that I have received and read the Disclosure set out above and this authorization.
I certify that I understand the documents that I have received.
I hereby authorize Quest Utility Construction Inc. or its authorized agents, for employment purposes, to obtain or prepare consumer reports at any time after it receives this authorization, including any time that I may be employed by Quest Utility Construction Inc.
I hereby authorize law enforcement agencies, public and private schools, federal, state and local agencies and courts, credit bureaus, information bureaus, current and former employers, financial institutions, licensing agencies, governmental agencies, the military, and other individuals and entities to provide any and all information that is requested by ExpressNet, other consumer reporting agencies or Quest Utility Construction Inc.. I also agree that a fax, scan, or photocopy of this authorization with my signature be accepted with the same authority as the original.
I understand that I have no guarantee of being hired by Quest Utility Construction Inc. and that Quest Utility may determine not to engage me for any lawful reason.
I certify that the information provided on this form is true and correct. I understand that any information that I provide in an employment application or that I otherwise disclose during my employment may be used to obtain consumer reports on me.
